Welcome to Acupuncture & Holistic Medicine, Inc. (“AHM,” “we,” “our,” or “us”). By scheduling an appointment, using our website, or receiving services from our clinic, you agree to the following Terms & Conditions. Please read them carefully.

1. Services Provided

AHM offers acupuncture, Reiki, massage therapy, herbal medicine, homeopathy, cupping, and related holistic and integrative wellness services. All treatments are provided by licensed or certified professionals in accordance with Florida law and applicable regulations.

2. Medical Disclaimer

Services provided at AHM are not a substitute for emergency medical care or diagnosis by a licensed physician.
Patients are responsible for informing their practitioner of all medical conditions, current medications, and any changes to their health status.
We do not guarantee specific outcomes; results vary depending on individual circumstances.

3. Appointments & Cancellations

Appointments may be booked online, by phone, or in person.
We request at least 24 hours’ notice for cancellations or rescheduling.
Missed appointments or late cancellations may be subject to a cancellation fee of $25.
Repeated missed appointments may result in denial of future booking privileges.

4. Payments & Insurance

Payment is due at the time of service unless other arrangements have been made.
We accept cash, checks, and major credit cards.
For patients using insurance, AHM will assist with benefit verification and, when applicable, bill your provider directly.
Patients remain financially responsible for all charges not covered by insurance, including deductibles, co-pays, co-insurance, visit limits, or services not included in their plan.
Packages and promotional offers are non-refundable and non-transferable unless required by law.

5. Refunds

No refunds will be issued for services already rendered.
Pre-paid treatment packages may be refunded on a prorated basis only in cases of documented medical necessity or relocation outside of service area, less any used sessions and applicable administrative fees.
Products (herbal formulas, supplements, etc.) are non-returnable once opened, unless defective.

6. Privacy & Confidentiality

AHM complies with all applicable privacy laws and maintains the confidentiality of patient information.
Personal health information will not be shared without written consent, except as required by law.

7. Website Use

Content on our website is for informational purposes only and should not be considered medical advice.
We make reasonable efforts to keep information accurate, but we do not guarantee completeness or suitability for any particular purpose.
External links are provided as a convenience; AHM is not responsible for third-party content.

8. Code of Conduct

Patients are expected to treat staff and practitioners with respect.
Any inappropriate, abusive, or unsafe behavior may result in termination of services.
Practitioners reserve the right to refuse service if treatment is deemed unsafe, inappropriate, or outside the scope of practice.

9. Limitation of Liability

By receiving services at AHM, you acknowledge that holistic therapies may involve mild risks (e.g., minor bruising, soreness, temporary fatigue). AHM, its practitioners, and staff shall not be liable for any damages beyond the cost of services rendered, except where prohibited by law.

10. Changes to Terms

AHM reserves the right to update these Terms & Conditions at any time. Changes will be posted on our website with a revised effective date. Continued use of our services constitutes acceptance of the updated terms.

11. Governing Law

These Terms & Conditions shall be governed by and construed in accordance with the laws of the State of Florida.
